By completing this form, you acknowledge that you are declining your offer of admission from the Master of Science in Quantitative Management (MSQM) program at the New York University Leonard N. Stern School of Business. This form acts as your formal notice that you will be declining your offer of admission to the MSQM program beginning in August 2022. If you have already paid your $750 enrollment deposit, you will forfeit this deposit and will not receive a refund.

If you wish to begin the program again in the future, you will need to submit a new application in a future application cycle and repay the application fee.
